Changing Name/Directory Information
If a student’s name, address, or telephone number changes at any time after initial enrollment, the student is required to complete the Student Change of Information Form. Proof of legal name change must accompany this form when changing a name for any reason. The form must include the student’s signature, and be returned to the Registrar's office.
